Dear Parent/Guardian,

As part of our Child-Parent Relationship Therapy Program, we may request you to upload videos via WeTransfer to facilitate our sessions. Please read the following information carefully and provide your consent.

Purpose of Video Uploads

The videos will be used solely for therapeutic purposes to enhance the therapy experience for you and your child. Only the therapist engaged in the program with you will view the videos. They will not be shared with anyone outside the organization- unless you specifically authorize such sharing. If you are happy for therapist to use for training purposes, you would be provided with an additional consent form to be signed.

  • Hosting Location: WeTransfer's servers are in stored and physically located in the EU.
  • Content Storage Duration: Files uploaded via WeTransfer are stored for a maximum of 30 days before being automatically deleted.

By signing below, you acknowledge that you understand the purpose of the video uploads and consent to the use of WeTransfer for this process and it is your responsibility to upload the video you choose to show therapist of your sessions with your child. Additionally, you agree to the terms of WeTransfer's privacy policy.

Playtonomy AI Policy and Consent

 

I understand that Playtonomy may use artificial intelligence (AI) tools such as ChatGPT to assist in writing clinical notes or reports related to my child’s care.

I acknowledge that:

  • All personal identifying information, including my child’s name, will be removed before any information is processed by AI.
  • AI will only be used to assist in drafting documents; all final content is reviewed and approved by qualified clinicians.
  • My consent is voluntary, and I can withdraw it at any time by informing the service.
  • The use of AI is intended to improve efficiency but does not replace clinical judgment or personalized care.

I give permission for Playtonomy to use AI tools for my child’s clinical documentation.
